1. When you schedule your appointment, we will ask for your insurance and demographic information. If your visit requires any lab tests, we will schedule those a few days before your appointment. We understand that this will require most of you to come into the office twice: once for lab tests and then for your appointment. Getting lab tests before the appointment allows Dr. Sharp to review your results with you during your visit. Only one co-payment is required: on the day of your appointment with the doctor.

2. You don't need to make an appointment for a blood draw, but please remember that the clinic is closed every Thursday afternoon. Mondays and Fridays are our busiest days, so be prepared for a short wait on those days before the Medical Assistant can take your blood.

3. Please be 15 minutes early for your appointment. When patients come early, we are better able to stay on schedule. We know that your time is valuable, but please be understanding if you have to wait. If you are more than 15 minutes late for your appointment, we may ask you to reschedule. Late patients affect all subsequent patients.

4. Schedule constraints require Dr. Sharp to visit with and examine only the person for whom the appointment is scheduled. If time allows him to see a second person who has not scheduled an appointment, he is ethically required to document that visit and contractually required to collect a second co-pay payment and to bill the insurance company for a second office visit.

5. We require 24-hour notice if you will be unable to keep your appointment so that another patient can use that time. We will bill you $25.00 for all missed, uncancelled appointments.
